For a copy of your medical records:

    • Please click the link below to submit a medical records request; Do NOT submit your request in person.
    • There is no charge for your records.
    • You should receive your records within 5 business days of completing your request.

CD/IMAGING REQUESTS: To request a CD, please call (406) 586-8029 and speak to any of the front office staff to put in your imaging request. The CD can be picked up in office or mailed.
